Although the use of insulin in diabetic pregnancies has resulted in dramatic improvements in maternal outcome, fetal and neonatal complications still present a challenge. Successful management depends on close monitoring of maternal glycaemic control pre- and post-conception, early diagnosis of gestational glucose intolerance, and careful assessment of fetal wellbeing. These aims are best achieved in combined obstetric/diabetic clinics.
