Imagery rehearsal therapy did not significantly reduce combat-related nightmares or PTSD symptoms in Vietnam veterans. The treatment showed no significant improvement over a comparison group for sleep quality or nightmare frequency.
Area of Science:
- Psychiatry and Behavioral Sciences
- Trauma and PTSD Research
- Sleep Medicine
Background:
- Chronic, severe posttraumatic stress disorder (PTSD) significantly impacts veterans' quality of life.
- Combat-related nightmares are a distressing symptom of PTSD, often resistant to treatment.
- Imagery rehearsal therapy (IRT) is a potential intervention for managing trauma-related nightmares.
Purpose of the Study:
- To evaluate the efficacy of imagery rehearsal therapy (IRT) in treating combat-related nightmares in male Vietnam War veterans.
- To compare the effects of IRT versus an active comparison condition on nightmare frequency, sleep quality, and PTSD symptoms.
Main Methods:
- A randomized controlled trial involving 124 male Vietnam War veterans with chronic, severe PTSD.
- Participants were assigned to either imagery rehearsal (n=61) or an active comparison condition (n=63).
- The intervention consisted of six group-based sessions.
Main Results:
- Both groups showed pre-post improvements in overall sleep quality and PTSD symptoms.
- Neither group experienced a significant reduction in nightmare frequency.
- Imagery rehearsal did not yield significantly greater improvements than the comparison condition for primary outcomes (nightmare frequency, sleep quality) or secondary outcomes (PTSD symptoms).
Conclusions:
- Six sessions of group-based imagery rehearsal therapy were not effective in substantially improving chronic, severe PTSD or combat-related nightmares in Vietnam veterans.
- The findings suggest that IRT, in this format and duration, may not be a sufficient intervention for this population.
- Further research is needed to explore potential explanations for the lack of efficacy and alternative treatment approaches.
