Summary

Surgeons experienced in radical retropubic prostatectomy can achieve excellent results in radical perineal prostatectomy with minimal learning curve. Positive margins significantly decreased, indicating improved surgical proficiency.

Area of Science:

  • Urology
  • Surgical Oncology
  • Medical Education

Background:

  • Radical retropubic prostatectomy is a standard treatment for prostate cancer.
  • Formal training in radical retropubic prostatectomy may influence the learning curve for radical perineal prostatectomy.

Purpose of the Study:

  • To determine the learning curve for radical perineal prostatectomy in surgeons trained in radical retropubic prostatectomy.
  • To assess the impact of experience on peri-operative outcomes.

Main Methods:

  • Analysis of peri-operative data from the William Beaumont Hospital Prostatectomy database.
  • Inclusion of 96 initial cases of radical perineal prostatectomy performed by two surgeons.
  • Evaluation of surgical outcomes including skin time, blood loss, length of stay, and positive margin rates.

Main Results:

  • Consistent and excellent peri-operative outcomes were observed over time for skin time, blood loss, and length of stay.
  • A significant decrease in positive margin rates was noted in the fourth quartile of cases.
  • These findings suggest a learning curve associated with achieving optimal oncological control.

Conclusions:

  • Excellent and reproducible results in radical perineal prostatectomy can be achieved using fundamental surgical techniques.
  • Formal training in radical retropubic prostatectomy facilitates a rapid and effective learning curve for radical perineal prostatectomy.
  • The study highlights the efficacy of basic surgical principles without reliance on advanced technology.
