Summary

Observers showed good overall agreement in measuring cup/disc ratios (CDR), but struggled with intermediate values. This highlights challenges in accurately assessing glaucoma progression using CDR simulations.

Area of Science:

  • Ophthalmology
  • Medical Imaging
  • Glaucoma Diagnosis

Background:

  • Accurate measurement of the cup-to-disc ratio (CDR) is crucial for diagnosing and monitoring glaucoma.
  • Simulations are used to train and assess observers' ability to measure CDR.

Purpose of the Study:

  • To evaluate and compare observer performance in measuring simulated cup-to-disc ratios (CDR) as both concentric and non-concentric circles.
  • To identify potential discrepancies in CDR measurements across different values and orientations.

Main Methods:

  • A prospective, randomized, and masked study involving 171 participants.
  • 43 simulated images of CDRs ranging from 0.2 to 0.9 were presented on a computer screen.
  • Both vertical and horizontal CDR measurements were assessed.

Main Results:

  • Satisfactory overall agreement was observed, with kappa coefficients of 0.755 (horizontal) and 0.730 (vertical).
  • Lin's concordance correlation coefficients were R=0.88 (horizontal) and R=0.86 (vertical).
  • Very poor agreement was found for intermediate CDR values (0.4-0.6), with kappa coefficients below 0.45.

Conclusions:

  • While general agreement for CDR measurement is adequate, absolute agreement for intermediate values is poor.
  • This indicates a significant challenge for observers in accurately quantifying CDR in the intermediate range.
  • Further research may be needed to improve observer training and measurement techniques for glaucoma assessment.
