Intracanalicular epidermoid cyst. A case report

• A rare case of an epidermoid cyst was identified within the left internal acoustic meatus.
• The cyst was located entirely within the petrous portion of the temporal bone.
• The cyst contained characteristic cheesy, pearly white, avascular material.

Findings:

• The patient underwent surgical excision using an extended middle fossa approach.
• Complete removal of the epidermoid cyst was successfully achieved.
• Neuroradiology accurately diagnosed the cyst and facilitated surgical planning.

Implications:

• This case highlights the importance of neuroradiology in diagnosing rare CPA lesions.
• The extended middle fossa approach is a viable surgical option for IAM epidermoid cysts.
• Successful total excision can lead to favorable patient outcomes.
