Background:

Primary lifelong delayed ejaculation (DE) remains understudied compared to other ejaculation disorders.
Currently, no specific drug treatments are approved for lifelong DE.

Purpose of the Study:

To investigate the clinical and laboratory features of men with primary lifelong DE.
To evaluate the efficacy of bupropion in treating lifelong DE.

Main Methods:

Prospective study of 19 men with primary lifelong DE, compared to 19 age-matched controls.
Evaluated International Index of Erectile Function (IIEF), anxiety/depression scores, ejaculation latency time (IELT), serum prolactin (PRL), and testosterone (T).
Patients received open-label bupropion-SR 150 mg/day for 2 months.

Main Results:

DE patients had higher masturbatory activity, lower night emissions, longer IELT, lower IIEF satisfaction scores, and higher anxiety/depression than controls.
Serum PRL and T levels were not significantly different between groups.
Bupropion showed limited improvement in ejaculation control (21.1% rated "fair to good"), with some improvements in IIEF satisfaction and depression scores.

Conclusions:

Lifelong DE is associated with specific behavioral patterns, infertility, longer IELT, lower satisfaction, and psychological distress.
Bupropion-SR at 150 mg/day demonstrated limited therapeutic benefit for lifelong DE.
