Describing shoulder (glenohumeral) motion accurately is difficult. The XZ'Y″ angle sequence provides a more comparable description to helical angles for arm elevation than the YX'Y″ sequence.
Area of Science:
- Biomechanics
- Human Movement Analysis
- Orthopedics
Background:
- Accurate description of glenohumeral motion is crucial for understanding shoulder function and dysfunction.
- Conventional Euler/Cardan angle sequences can present singularities, limiting their utility across the full range of motion.
- The International Society of Biomechanics (ISB) recommends specific angle sequences, but alternatives may offer advantages.
Purpose of the Study:
- To compare the description of glenohumeral motion using the ISB-recommended YX'Y″ sequence versus an alternative XZ'Y″ sequence.
- To evaluate the suitability of these sequences for analyzing dynamic, in-vivo shoulder movement in the scapular plane.
Main Methods:
- A direct in-vivo analysis of dynamic concentric glenohumeral joint motion was performed.
- An electromagnetic tracking system recorded data from ten healthy individuals during arm elevation in the scapular plane.
- Angular position data from the YX'Y″ and XZ'Y″ sequences were compared.
Main Results:
- Significant differences were observed in the description of angular position data between the two sequences.
- The YX'Y″ sequence indicated greater anterior and external rotation of the humerus compared to the XZ'Y″ sequence, particularly at lower elevation angles.
- The XZ'Y″ sequence's motion description closely matched helical angles for arm elevation, unlike the YX'Y″ sequence.
Conclusions:
- The XZ'Y″ angle sequence offers a more consistent and comparable description of glenohumeral motion, especially during arm elevation in the scapular plane.
- The YX'Y″ sequence's description of motion direction can be contrary to helical angle analysis.
- The XZ'Y″ sequence is recommended as a viable alternative for accurately describing complex glenohumeral joint movements.
