Background:

  • Cardiac arrhythmias are a significant cause of morbidity and mortality in patients undergoing non-cardiac vascular surgery.
  • Perioperative cardiovascular complications pose a substantial risk in this patient population.

Observation:

  • An implantable loop recorder (Reveal® XT) was utilized for continuous heart rhythm monitoring during major vascular surgery.
  • The device detected asymptomatic new-onset atrial fibrillation postoperatively in a 69-year-old male patient.
  • Elevated serum levels of troponin-T and N-terminal pro-B-type natriuretic peptide (NT-proBNP) were observed concurrently.

Findings:

  • Continuous monitoring identified previously undetected perioperative atrial fibrillation.
  • Asymptomatic arrhythmias can occur postoperatively in vascular surgery patients.
  • Correlation between arrhythmias and elevated cardiac biomarkers (troponin-T, NT-proBNP) was noted.

Implications:

  • Continuous heart rhythm monitoring can facilitate early identification of high-risk patients.
  • Assessment of serum cardiac biomarkers alongside rhythm monitoring aids in risk stratification.
  • Prompt diagnosis and treatment of perioperative arrhythmias may improve patient outcomes and reduce mortality.
