Summary

Mycoplasma pneumoniae pneumonia can be difficult to diagnose initially. Prompt diagnosis and appropriate antibiotic treatment with minocycline or doxycycline led to full recovery in two challenging cases.

Background:

  • Mycoplasma pneumoniae is a common cause of community-acquired pneumonia.
  • Initial antibiotic choices may not be effective against Mycoplasma pneumoniae.
  • Diagnosis can be delayed if this etiology is not suspected early.

Observation:

  • Two patients presented with pneumonia unresponsive to initial broad-spectrum antibiotics (cefamandole, carbenicillin, cephradine, gentamycin).
  • High serum antibody titers (1:2560) indicated Mycoplasma pneumoniae infection.
  • Clinical improvement was observed after switching to tetracycline-class antibiotics.

Findings:

  • Minocycline (200 mg/day for 16 days) resulted in clinical improvement within two days for the first patient.
  • Doxycycline (200 mg/day for 10 days) led to recovery in the second patient after ten days.
  • Both patients achieved full recovery and were discharged.

Implications:

  • Highlights the importance of considering Mycoplasma pneumoniae in refractory pneumonia cases.
  • Suggests minocycline and doxycycline as effective treatment options for Mycoplasma pneumoniae pneumonia.
  • Emphasizes the role of serological testing in confirming diagnosis and guiding therapy.
