Drugs for Treatment of Ulcerative Colitis in IBD01:29

Drugs for Treatment of Ulcerative Colitis in IBD

Ulcerative colitis is a chronic inflammatory condition primarily affecting the colon and rectum. The primary drugs used in the treatment of ulcerative colitis are aminosalicylates. They exhibit anti-inflammatory and immunosuppressive properties. They modulate inflammatory mediators and inhibit the activity of nuclear factor κB (NF-κB). Aminosalicylates also reduce inflammation by inhibiting prostaglandin and leukotriene production and decreasing neutrophil chemotaxis and superoxide generation.

Antiasthma Drugs: Mast Cell Stabilizers and Anti-IgE Drugs

Asthma is a chronic respiratory condition for which new therapeutic avenues, including anti-inflammatory drugs like mast cell stabilizers and anti-IgE treatments, continue to be developed.
Mast cell stabilizers, such as cromolyn (also known as sodium cromoglycate) and nedocromil (Tilade), are effective drugs in asthma management. These stabilizers hinder histamine release by skillfully obstructing the activation of mast cells and other cellular entities. Notably, they navigate this task without...
Antiasthma Drugs: Leukotriene Modifiers01:19

Antiasthma Drugs: Leukotriene Modifiers

Leukotriene modifiers, or cysteinyl leukotriene receptor antagonists, are medications used to manage chronic asthma. These agents target specific inflammatory mediators produced during arachidonic acid metabolism, an essential process in generating inflammation in the body.
Leukotriene modifiers work through two distinct mechanisms:

Drugs for Treatment of Crohn's Disease in IBD Using Biologic Agents: Anti-TNF

Tumor Necrosis Factor (TNF), a proinflammatory cytokine, contributes significantly to the inflammation seen in Crohn's disease. It exists as soluble TNF and membrane-bound TNF, with actions mediated through TNF receptors (TNFR). TNFR activation leads to the release of proinflammatory cytokines, T-cell activation, collagen production, and leukocyte migration, all contributing to inflammation in Crohn's disease. Anti-TNF monoclonal antibodies, namely infliximab (Remicade), adalimumab (Humira),...

Inflammatory Bowel Disease IV: Pharmacological Management

Upon diagnosis, managing Inflammatory Bowel Disease (IBD) involves addressing several crucial aspects. The primary goals include resting the bowel, correcting malnutrition, and providing symptomatic relief. Resting the bowel may consist of medications to reduce inflammation and promote healing. Correcting malnutrition is essential, often requiring dietary adjustments and nutritional supplements. Symptomatic relief aims to ease pain, diarrhea, and other discomforts in IBD.
Pharmacologic...

Summary
This summary is machine-generated.

Efalizumab, a biologic therapy for psoriasis, showed initial promise but was withdrawn due to severe adverse events like progressive multifocal leukoencephalopathy in long-term users. Post-marketing surveillance is crucial for evaluating drug safety.

Area of Science:

  • Immunodermatology
  • Pharmacovigilance
  • Biologic Therapies

Background:

  • Conventional systemic psoriasis treatments pose significant toxicity risks, limiting long-term use.
  • Biologic therapies offer improved safety and efficacy for psoriasis management.
  • Efalizumab, a T-cell modulating biologic, targets key stages in psoriasis immunopathogenesis.

Purpose of the Study:

  • To review the efficacy and safety of efalizumab for moderate-to-severe chronic plaque psoriasis.
  • To highlight the critical role of post-marketing surveillance in drug evaluation.

Main Methods:

  • Review of clinical trial data demonstrating efalizumab's efficacy and quality-of-life benefits.
  • Analysis of post-marketing reports, including cases of progressive multifocal leukoencephalopathy.

Main Results:

  • Efalizumab demonstrated efficacy in treating moderate-to-severe chronic plaque psoriasis.
  • Long-term efalizumab therapy (>3 years) was associated with severe adverse events, including progressive multifocal leukoencephalopathy.
  • These safety concerns led to the suspension of efalizumab's marketing authorization.

Conclusions:

  • While initially effective, efalizumab's long-term safety profile was compromised by severe adverse events.
  • This case underscores the necessity of robust post-marketing surveillance for comprehensive drug safety assessment.
