Drugs for Treatment of Ulcerative Colitis in IBD

Ulcerative colitis is a chronic inflammatory condition primarily affecting the colon and rectum. The primary drugs used in the treatment of ulcerative colitis are aminosalicylates. They exhibit anti-inflammatory and immunosuppressive properties. They modulate inflammatory mediators and inhibit the activity of nuclear factor κB (NF-κB). Aminosalicylates also reduce inflammation by inhibiting prostaglandin and leukotriene production and decreasing neutrophil chemotaxis and superoxide generation.

  • Patients often face prolonged intensive care unit (ICU) stays with uncertain outcomes.

Purpose of the Study:

  • Develop a predictive risk score for perioperative mortality in AIC patients.
  • Examine long-term survival outcomes for patients with AIC.

Main Methods:

  • Prospective study of 177 patients undergoing surgical treatment for AIC (2002-2008).
  • Uni- and multivariate analyses identified independent predictors of mortality and survival.

Main Results:

  • A risk score incorporating nonocclusive AIC, renal failure, ischemia extent, lactate, and catecholamine duration predicted postoperative mortality.
  • Mesenteric atherosclerosis in surgical specimens independently predicted poor long-term survival (52 vs 40 months, P = .027).

Conclusions:

  • A novel, highly predictive risk score for postoperative mortality in surgically treated AIC patients was developed.
  • The score can aid in selecting and modifying therapeutic management for AIC patients.
  • Mesenteric atherosclerosis significantly influences long-term survival in AIC patients.
