Summary

Primary hepatic carcinosarcoma, a rare liver cancer with mixed elements, presents diagnostic challenges due to tumor heterogeneity. Accurate preoperative diagnosis is crucial for effective treatment of this uncommon malignancy.

Background:

  • Primary hepatic carcinosarcoma is a rare liver malignancy with both carcinomatous and sarcomatous components.
  • Limited data exists on its risk factors, epidemiology, pathogenesis, and treatment.

Observation:

  • A case of primary hepatic carcinosarcoma in a 69-year-old male presenting with abdominal pain and weight loss.
  • Imaging revealed a large liver mass with vena hepatica invasion.

Findings:

  • Biopsy showed osteoid tissue, potentially misdiagnosed as metaplastic bone.
  • Intraoperative frozen sections confirmed carcinosarcoma with osteosarcoma and cholangiocellular carcinoma elements.

Implications:

  • Intratumoral heterogeneity can complicate preoperative diagnosis via biopsy.
