Summary

A rare uretero-aortic fistula developed after prolonged ureteral stenting in a patient with a history of advanced cancers. This led to massive bleeding, successfully treated by surgical repair and nephroureterectomy.

Background:

A 59-year-old woman with advanced cervical and bladder cancers underwent radical hysterectomy and total cystectomy with a single stoma cutaneous ureterostomy.
Postoperative pyelonephritis due to bilateral ureteral stenosis was managed with bilateral ureteral stenting.

Observation:

The patient experienced intermittent hematuria from the left ureterostomy for four years following ureteral stenting.
Massive bleeding occurred, necessitating significant blood transfusion, and CT scans revealed severe adhesion of the left ureter to the aorta.

Findings:

A uretero-aortic fistula, approximately 5 mm in diameter, was identified during surgical exploration.
The fistula was attributed to prolonged ureteral stenting and the ureter's anatomical course over the aorta.

Implications:

This case highlights a rare but potentially life-threatening complication of ureteral stenting.
Prompt diagnosis and surgical intervention are crucial for managing uretero-aortic fistulas.
