Summary

A rare uretero-aortic fistula developed after prolonged ureteral stenting in a patient with a history of advanced cancers. This led to massive bleeding, successfully treated by surgical repair and nephroureterectomy.

A Novel Murine Model of Arteriovenous Fistula Failure: The Surgical Procedure in Detail

Background:

  • A 59-year-old woman with advanced cervical and bladder cancers underwent radical hysterectomy and total cystectomy with a single stoma cutaneous ureterostomy.
  • Postoperative pyelonephritis due to bilateral ureteral stenosis was managed with bilateral ureteral stenting.

Observation:

  • The patient experienced intermittent hematuria from the left ureterostomy for four years following ureteral stenting.
  • Massive bleeding occurred, necessitating significant blood transfusion, and CT scans revealed severe adhesion of the left ureter to the aorta.

Findings:

  • A uretero-aortic fistula, approximately 5 mm in diameter, was identified during surgical exploration.
  • The fistula was attributed to prolonged ureteral stenting and the ureter's anatomical course over the aorta.

Implications:

  • This case highlights a rare but potentially life-threatening complication of ureteral stenting.
  • Prompt diagnosis and surgical intervention are crucial for managing uretero-aortic fistulas.
