Summary

Subacute thyroiditis, a transient hyperthyroid condition, can present with neck pain and elevated inflammatory markers. Prompt steroid treatment offers symptom relief, followed by potential hypothyroidism.

Area of Science:

Background:

  • A 41-year-old woman presented with persistent neck pain and malaise after antibiotic treatment for a sore throat.
  • She had a history of moderate hyperthyroidism treated with antithyroid drugs.

Observation:

  • Physical examination revealed a tender, firm thyroid gland without enlargement.
  • Clinical signs and symptoms were consistent with moderate hyperthyroidism.

Findings:

  • Laboratory tests showed suppressed TSH, elevated thyroid hormones, elevated erythrocyte sedimentation rate, and CRP.
  • Thyroid ultrasound demonstrated a normal-sized but severely hypoechoic gland.
  • A thyroid scan indicated low technetium uptake.

Implications:

  • The diagnostic findings supported subacute thyroiditis.
