A fluorescence microscope uses fluorescent chromophores called fluorochromes, which can absorb energy from a light source and then emit this energy as visible light. Fluorochromes include naturally fluorescent substances (such as chlorophylls) and fluorescent stains that are added to the specimen to create contrast. Light chain immunofluorescence (IF) in kidney biopsies is crucial for diagnosing various nephropathies. While lambda light chain predominance is seen in IgA nephropathy, kappa is more common in light chain deposition disease (LCDD).

Background:

  • Renal biopsy immunofluorescence (IF) is standard for diagnosing light chain deposition disease (LCDD), amyloidosis, and cast nephropathy.
  • Light chain predominance is observed in glomerulopathies like IgA nephropathy, but its pathogenic role remains unclear.

Purpose of the Study:

  • To elucidate the pathogenesis of light chain deposition in various nephropathies.
  • To evaluate the diagnostic utility of light chain immunofluorescence (IF) in renal biopsies.

Main Methods:

  • Retrospective analysis of 306 renal biopsy cases.
  • Review of light microscopic diagnoses and light chain IF patterns.
  • Direct IF utilized anti-kappa and anti-lambda antisera.

Main Results:

  • Light chain deposits were identified in 78.43% of cases.
  • Lambda predominance observed in IgA nephropathy, lupus nephritis, and post-infectious glomerulonephritis (PIGN).
