Transperineal laparoscopy is a new surgical approach for men undergoing prostatectomy and cystectomy. This technique shows promise as a feasible alternative, with good outcomes in initial patient evaluations.

Purpose of the Study:

  • To describe and evaluate a novel transperineal laparoscopic approach for male patients.
  • To assess the feasibility of this technique for laparoscopic prostatectomy and cystectomy.

Main Methods:

  • A transperineal incision was made, creating a space between the rectum and prostate.
  • A cannula was inserted into the peritoneal cavity via the Douglas pouch.
  • The technique was applied to 12 male patients undergoing prostatectomy or cystectomy.

Main Results:

  • Of 12 patients, 3 underwent prostatectomy and 9 underwent cystectomy (5 ileal conduit, 4 orthotopic bladder).
  • One prostatectomy patient required incontinence pads; no daytime incontinence was observed in orthotopic bladder cases.
  • One urethral injury occurred during ileal conduit formation; no other major complications were reported.

Conclusions:

  • Transperineal laparoscopy is a feasible surgical technique in male patients.
  • This approach may serve as an alternative to natural orifice translumenal endoscopic surgery (NOTES) in women.
  • Further research is needed to fully establish its role and benefits.
