Summary

Giant abdominal aortic aneurysms (AAAs) rarely shrink spontaneously. This case report details a 9-cm AAA that significantly regressed to 4 cm over 7 years without intervention, a phenomenon previously undescribed.

Background:

  • Abdominal aortic aneurysms (AAAs) are a significant cause of morbidity and mortality.
  • Giant AAAs (>5.5 cm) typically require intervention due to high rupture risk.
  • Spontaneous regression of AAA sac size has not been previously documented.

Observation:

  • An 89-year-old woman presented with a known giant AAA measuring 9 cm in 2003.
  • The patient declined any form of treatment for the AAA.
  • Follow-up imaging 7 years later showed a remarkable reduction in AAA diameter to 4 cm.

Findings:

  • This case represents the first documented instance of significant spontaneous regression in abdominal aortic aneurysm sac size.
  • The observed regression challenges the conventional understanding of AAA natural history.
  • The mechanism underlying this spontaneous shrinkage remains unknown.

Implications:

  • This finding may prompt further research into the biological mechanisms of aneurysm regression.
  • Understanding spontaneous regression could potentially lead to novel, non-invasive therapeutic strategies for AAAs.
  • Further investigation is warranted to determine if this phenomenon can be replicated or induced.
