Summary

Primary hyperparathyroidism (pHPT) with normal parathyroid hormone (PTH) levels shows similar symptoms to elevated PTH. However, normal PTH pHPT patients have smaller adenomas and require more extensive surgery.

Background:

  • Primary hyperparathyroidism (pHPT) is a common endocrine disorder.
  • A subset of pHPT patients present with normal parathyroid hormone (PTH) levels, previously considered a milder form.

Purpose of the Study:

  • To investigate the clinical characteristics, laboratory values, imaging findings, and surgical outcomes of sporadic pHPT patients with normal PTH levels.
  • To compare these features against a matched cohort of pHPT patients with elevated PTH levels.

Main Methods:

  • A retrospective review of 861 sporadic pHPT patients who underwent parathyroidectomy.
  • Comparison of 58 patients with normal PTH (within 6 months pre-surgery) against 116 matched controls with elevated PTH.

Main Results:

  • No significant differences in preoperative calcium, symptoms, or bone health between normal and elevated PTH groups.
  • Normal PTH group had higher vitamin D, smaller adenomas, and more frequent bilateral neck exploration.
  • Preoperative imaging sensitivity trended lower in the normal PTH group.

Conclusions:

  • pHPT with normal or elevated PTH levels present similarly regarding symptoms and calcium.
  • Normal PTH pHPT is often associated with single-gland disease and smaller adenomas.
  • These factors may contribute to less sensitive imaging and increased need for four-gland exploration.
