Background:

Complete talar extrusion is a rare and severe foot trauma.
Treatment options for talar extrusion are controversial due to limited evidence and high complication rates.
Assessing long-term outcomes of talar extrusion injuries is challenging.

Observation:

A rare case of complete talar extrusion following trauma is presented.
The extruded talus was fractured.
The patient underwent immediate talus reimplantation.

Findings:

Complete revascularization of the reimplanted talus was confirmed.
Magnetic resonance imaging at 1-year follow-up demonstrated successful revascularization.
This case supports the viability of immediate talus reimplantation for severe extrusion injuries.
