Background:

  • Accurate differentiation between benign and malignant hepatic lesions is crucial for appropriate patient management.
  • Traditional morphologic analysis of hepatic lesions can sometimes be ambiguous.

Purpose of the Study:

  • To evaluate quantitative magnetic resonance imaging (MRI) methods for distinguishing benign hepatic cavernous hemangiomas from malignant hepatic neoplasms.
  • To identify the most effective quantitative image analysis technique for this diagnostic challenge.

Main Methods:

  • Analysis of MRI scans (0.6 T) from 42 patients with hepatic cavernous hemangioma and 63 patients with hepatic malignancies.
  • Calculation of lesion-liver signal-intensity ratios using specific pulse sequences (repetition time = 2,350 msec, echo time = 180 msec).

  • The lesion-liver signal-intensity ratio demonstrated high accuracy in differentiating hemangiomas from cancers (Area Under the ROC Curve = 0.99 +/- 0.005).
  • Ratios > 3.5 exclusively indicated hemangiomas (38/38), while ratios < 2.5 exclusively indicated malignant neoplasms (57/57).
  • Quantitative signal-intensity data alone achieved 93% specificity and 89% sensitivity.

Conclusions:

  • Signal-intensity ratios are a valuable quantitative tool for the differential diagnosis of hepatic lesions.
  • This method, combined with T2-weighted imaging, significantly enhances diagnostic accuracy beyond morphologic assessment.
