Summary

Photodynamic therapy can cause choroidal hypoperfusion and worsen wet age-related macular degeneration (AMD). Intravitreal anti-VEGF injections may help stabilize vision in these cases.

Background:

Photodynamic therapy (PDT) is a selective treatment for choroidal neovascularization (CNV).
Reports indicate potential choroidal hypoperfusion following PDT.
This case examines PDT-induced circulatory disturbances and CNV progression.

Observation:

A 75-year-old woman with occult CNV underwent initial PDT.
Post-PDT, visual acuity decreased, and central choroidal hypoperfusion with subretinal fluid was noted.
This led to neovascular age-related macular degeneration (AMD) progression.

Findings:

PDT can induce choroidal circulation disturbances, potentially worsening neovascular AMD.
Intravitreal anti-VEGF therapy (ranibizumab) was administered over 12 months.
Treatment resulted in regression of wet AMD activity and improved visual acuity.

Implications:

Choroidal hypoperfusion post-PDT is a possible complication requiring consideration.
Such disturbances can acutely decrease visual acuity and worsen the local ocular state.
Intravitreal anti-VEGF agents are a viable treatment option for neovascular AMD following PDT-induced complications, after stabilization.
