Area of Science:

  • Ophthalmology
  • Infectious Diseases
  • Rheumatology

Background:

  • Posterior scleral tuberculoma is exceptionally rare, with most tuberculosis-related scleral reports involving anterior scleritis.
  • Rheumatoid arthritis patients are susceptible to various ocular complications.

Observation:

  • A patient with rheumatoid arthritis developed a large posterior scleral mass.
  • The lesion led to retinal detachment and significant visual impairment.

Findings:

  • The posterior scleral mass was ultimately diagnosed as a tuberculoma.
  • Initial misdiagnosis as choroidal melanoma complicated the diagnostic process.

Implications:

  • This case underscores the importance of considering tuberculosis in atypical scleral presentations, even in the posterior segment.
