A comprehensive review of hip arthroscopy techniques and outcomes.

SAGE open medicine·2024

Area of Science:

  • Orthopedic Surgery
  • Sports Medicine
  • Trauma Surgery

Background:

  • Traumatic posterior hip dislocation often results from high-energy impacts.
  • Associated injuries may require management post-reduction.

Purpose of the Study:

  • To document arthroscopic findings in patients with mechanical hip symptoms after traumatic posterior hip dislocation.
  • To evaluate outcomes of hip arthroscopy in this patient cohort.

Main Methods:

  • Retrospective case series (Level of evidence, 4) of patients undergoing hip arthroscopy.
  • Analysis of arthroscopic findings, treatments, and patient-reported outcomes (WOMAC scores) post-dislocation and closed reduction.

Main Results:

  • Seventeen patients (average age 28.5 years) underwent arthroscopy a median of 3 months post-reduction.

  • Common findings included labral tears, acetabular and femoral chondral damage, and intra-articular fragments.
  • Significant improvement in WOMAC scores (46 to 87, P = .001) was observed over an average 45-month follow-up.
  • Conclusions:

    • Arthroscopy is indicated for mechanical hip symptoms after traumatic posterior hip dislocation, particularly when loose fragments are present.
    • Intra-articular damage is consistently found.
    • Hip arthroscopy generally leads to significant symptom improvement.
