Tracheoinnominate artery fistula (TIF) is a rare but fatal complication after tracheostomy. Early diagnosis and intervention, including ultrasound and tracheal monitoring, are crucial for survival.

- Diagnosis of TIF was challenging in both cases, with tracheal ring fracture and cannula-induced pressure ulceration implicated in fistula formation.
Findings:
- Overblocking the cannula was used as a temporizing measure before surgical repair in both patients.
- Both patients survived the TIF without developing neurological deficits.
Implications:
- Massive hemorrhage post-tracheostomy should raise suspicion for TIF.
- Pre-procedural ultrasound and regular tracheal follow-up are recommended to prevent and manage TIF.
