Background:

Dermatofibroma (DF) typically presents as a benign skin neoplasm confined to the dermis.
While subcutaneous extension is known, purely subcutaneous lesions are infrequent.
Understanding DF variants is crucial for accurate diagnosis and management.

Observation:

A 41-year-old male presented with a painless, firm, subcutaneous papule on his left thigh.
The lesion was completely excised following an incision.
Clinical presentation suggested a subcutaneous nodule.

Findings:

Histopathological examination confirmed the diagnosis of a subcutaneous dermatofibroma.
The lesion lacked typical epidermal changes associated with dermal DF.
This finding highlights a rare morphologic variant of dermatofibroma.

Implications:

This case expands the spectrum of clinical and histopathological presentations of dermatofibroma.
Recognition of purely subcutaneous DF is important to avoid misdiagnosis of other subcutaneous neoplasms.
Further studies may elucidate the specific etiology and behavior of this variant.
