Platelet (PLT) apheresis procedures, Amicus and Trima Accel, were compared. Trima Accel demonstrated higher PLT recruitment and yield, facilitating multiple unit harvests from a single donation.

Area of Science:

  • Transfusion Medicine
  • Hematology
  • Apheresis Technology

Background:

  • Platelet (PLT) apheresis is crucial for blood component therapy.
  • Optimizing PLT apheresis procedures can enhance the efficiency of collecting multiple units per donation.
  • Understanding PLT recruitment dynamics is key to improving apheresis outcomes.

Purpose of the Study:

  • To compare the efficacy of two automated platelet apheresis systems, Amicus and Trima Accel.
  • To evaluate platelet (PLT) yields and PLT recruitment in donors undergoing apheresis.
  • To assess the impact of different apheresis procedures on donor platelet depletion and white blood cell counts.

Main Methods:

  • A prospective, randomized, paired cross-over study involving 60 donors.
  • Comparison of 120 apheresis donations using Amicus and Trima Accel systems.

  • Analysis of pre- and post-apheresis platelet counts, total PLT yield, and PLT recruitment (defined as ratio of total PLT yield to donor PLT depletion).
  • Main Results:

    • Both Amicus and Trima Accel induced platelet recruitment, with Trima Accel showing significantly higher recruitment (1.90 vs. 1.42, p < 0.001).
    • Trima Accel resulted in higher platelet yields (7.48 × 10(11) vs. 6.06 × 10(11), p < 0.001) despite comparable donor platelet depletion.
    • A more pronounced increase in white blood cell (WBC) counts was observed post-apheresis with Trima Accel compared to Amicus (1.30 × 10(9)/l vs. 0.46 × 10(9)/l, p < 0.001).

    Conclusions:

    • The Trima Accel system demonstrates superior platelet recruitment and yield compared to the Amicus system.
    • Platelet recruitment facilitates the collection of multiple units from a single donation, influenced by the apheresis procedure.
    • Further investigation is warranted regarding the differential increases in circulating donor white blood cells observed after each procedure.
