Summary

This case report details vision loss after endophthalmitis treatment. Spectral-domain optical coherence tomography (SD-OCT) revealed inner retinal damage from intravitreal gentamicin, with the outer retina remaining intact.

  • Intravitreal antibiotics are a standard treatment for endophthalmitis.

Observation:

  • A 75-year-old woman developed acute endophthalmitis post-phacoemulsification.
  • Treatment with vancomycin, ceftazidime, and gentamicin resulted in persistent visual loss.
  • Ocular examination showed a pale disc, retinal hemorrhages, and nerve fiber layer infarcts.

Findings:

  • Fluorescein angiography indicated macular vascular non-perfusion.
  • Spectral-domain optical coherence tomography (SD-OCT) demonstrated diffuse retinal thinning and absence of foveal depression.
  • SD-OCT revealed increased reflectivity in the nerve fiber and ganglion cell layers, with an intact inner segment/outer segment junction.

Implications:

  • This case highlights potential macular infarction following intravitreal gentamicin use.
  • SD-OCT imaging is crucial for characterizing inner retinal changes in such cases.
  • Understanding these specific SD-OCT findings aids in diagnosing and managing post-infectious or treatment-related retinal damage.
