Personality disorder functioning styles, not other symptoms, were linked to alexithymia in paranoid schizophrenia patients. This suggests a cognitive-personological basis for the disorder.
Area of Science:
- Psychiatry
- Clinical Psychology
- Cognitive Neuroscience
Background:
- Alexithymia, characterized by difficulties in identifying and describing emotions, has shown inconsistent findings in schizophrenia.
- Personality disorder functioning styles may influence the presentation and study of alexithymic features in schizophrenia.
Purpose of the Study:
- To investigate the relationship between alexithymia and personality disorder functioning styles in individuals with paranoid schizophrenia.
Main Methods:
- Employed Chinese versions of the Toronto Alexithymia Scale (TAS-20) and the Parker Personality Measure (PERM).
- Administered the Positive and Negative Syndrome Scale and Hamilton Anxiety and Depression Scales.
- Compared 60 paranoid schizophrenia patients with 60 healthy controls.
Main Results:
- Schizophrenia patients exhibited higher scores on the Positive and Negative Syndrome Scale, TAS subscales ('difficulty identifying feelings,' 'difficulty describing feelings'), Hamilton Depression Scale, and most PERM scales.
- In patients, difficulty identifying feelings predicted all PERM scales, while specific personality styles (antisocial, avoidant, histrionic, paranoid) predicted distinct alexithymia facets.
- In healthy controls, difficulty identifying feelings predicted a dependent style, and anxiety predicted difficulties in identifying and describing feelings.
Conclusions:
- Personality disorder functioning styles, rather than anxiety, depression, psychotic symptoms, or disease duration, were specifically associated with alexithymia in paranoid schizophrenia.
- These findings highlight a potential cognitive-personological substrate in paranoid schizophrenia.
- Further longitudinal research is recommended to understand the role of premorbid or residual personality styles in the disorder.
