Summary

Oral contraceptives were linked to cerebral venous thrombosis in a young woman, causing seizures and hemorrhages. Prompt anticoagulant therapy led to complete recovery and vessel recanalization.

Background:

Observation:

  • A 21-year-old woman using oral contraceptives presented with severe neurological symptoms including headache, nausea, vomiting, somnolence, and a generalized tonic-clonic seizure.
  • Cranial imaging revealed multiple cerebral hemorrhages.
  • MR venography demonstrated extensive dural sinus and venous thrombosis, affecting major cerebral veins.

Findings:

  • Anticoagulant treatment was initiated for the extensive cerebral venous thrombosis.
  • The patient experienced a complete neurological recovery within two weeks.
  • Complete recanalization of all thrombosed cerebral sinuses and veins was confirmed on follow-up imaging.

Implications:

  • This case highlights the potential for severe neurological complications from oral contraceptive-associated CVT.
  • Prompt anticoagulation can lead to excellent outcomes, even in cases with initial poor prognostic indicators.
  • Successful recanalization of cerebral veins is associated with complete neurological recovery.
