A novel epiglottic repositioning technique successfully corrected supraglottic stenosis/collapse in patients with failed decannulation. This procedure enabled successful decannulation in all treated cases, offering a solution for this uncommon condition.
Area of Science:
- Otolaryngology
- Surgical Innovation
Background:
- Supraglottic stenosis/collapse is an uncommon cause of failed decannulation after tracheostomy.
- Accurate diagnosis can be challenging.
Observation:
- A retrospective analysis identified 3 cases of supraglottic stenosis/collapse with posterior epiglottic displacement among 8 decannulation failures.
- These cases were treated with a novel epiglottic repositioning procedure.
Findings:
- The epiglottic repositioning technique involved an external incision to divide and reattach the epiglottis.
- Successful decannulation was achieved in all 3 patients who underwent the procedure.
Implications:
- This technique offers a viable solution for supraglottic stenosis/collapse.
- Diagnosis-directed laryngoscopy is crucial for identifying this condition.
- Further research may validate this surgical approach for decannulation failures.
