Summary
Parvovirus B19 (PVB19) infection can cause atypical purpuric eruptions beyond the typical gloves-and-socks presentation. Early diagnosis is crucial as these eruptions, termed PVB19-associated purpuric-petechial eruption, are self-limiting.

Background:
- Papular-purpuric gloves-and-socks syndrome (PPGSS) is a rare dermatosis linked to parvovirus B19 (PVB19) infection.
- Atypical presentations of PVB19-associated purpuric eruptions are increasingly reported, causing diagnostic challenges.
Observation:
- Two febrile patients presented with atypical PVB19-related purpuric eruptions.
- One patient had asymmetrical "gloves without socks" distribution with intertriginous involvement; the other had generalized, prominent intertriginous and truncal involvement.
- Both cases were preceded by erythema infectiosum.
Findings:
- Serologic tests and real-time PCR confirmed PVB19 infection.
- The eruptions correlated with the viremic phase of primary PVB19 infection.
- Histological findings were common across presentations.
Implications:
- PVB19 infection should be considered for any unexplained petechial or purpuric eruption, not just PPGSS.
- The term "PVB19-associated purpuric-petechial eruption" is proposed for these polymorphous eruptions.
- These eruptions have a self-limiting course, benign prognosis, and coincide with infectivity.
