Muscles of the Shoulder01:23

Muscles of the Shoulder

The muscles surrounding the shoulder girdle, including the clavicle and scapula, primarily stabilize the scapula. This stable base allows other muscles to move the humerus effectively. Scapular movements often mirror those of the humerus and extend its range of motion. For instance, raising the arm above the head would not be feasible without simultaneous upward rotation of the scapula.

Summary
This summary is machine-generated.

Posterior shoulder dislocations (PSDs) are challenging due to limited evidence. This review offers a treatment algorithm for PSDs, including minimally invasive options, to guide physicians.

Area of Science:

  • Orthopedic Surgery
  • Sports Medicine
  • Traumatology

Background:

  • Posterior shoulder dislocations (PSDs) represent a small fraction of shoulder dislocations.
  • Evidence-based treatment protocols and definitive algorithms for PSD management are scarce in medical literature.

Purpose of the Study:

  • To conduct a systematic literature review on posterior shoulder dislocations.
  • To provide an overview of diagnostic techniques for PSDs.
  • To present a treatment algorithm for PSDs, incorporating minimally invasive approaches.

Main Methods:

  • A systematic search of Medline and Cochrane databases was conducted for articles published between 1940 and 2010.
  • Studies with a minimum of 4 operated patients and documented functional outcomes were included.
  • Exclusion criteria included abstracts, radiologic reports, reviews, case reports, and technical notes. Level IV evidence was considered.

Main Results:

  • The review included 5 prospective case series and 6 retrospective studies, all of Level IV evidence.
  • Significant heterogeneity and limited patient numbers were noted across studies.
  • Descriptive comparisons of surgical techniques and case reports illustrating various approaches were presented.

Conclusions:

  • Posterior shoulder dislocations remain a clinical challenge with limited high-quality evidence.
  • The proposed algorithm offers decision-making guidance for PSD treatment.
  • The algorithm emphasizes the inclusion of minimally invasive treatment options based on current literature.
