Background:

Evaluating the growth of practice-based research networks (PBRNs) presents significant challenges.
Effective planning and evaluation are crucial for PBRN success and sustainability.

Purpose of the Study:

To describe the development of a logic model for a primary care PBRN.
To illustrate how this logic model framework facilitates planning and evaluation.

Main Methods:

Formed an evaluation team comprising PBRN directors, staff, and board members.
Conducted facilitated stakeholder meetings to identify assumptions, inputs, activities, outputs, outcomes, and indicators.
Defined the PBRN's mission and target audience.

Main Results:

The logic model established two long-term outcomes: improved patient health and clinician recognition as research leaders.
The model highlighted stakeholder interests and dissemination activities as areas needing enhanced focus.
The logic model approach proved valuable for PBRN planning and project management.

Conclusions:

Logic models are effective tools for planning and evaluating PBRNs.
This framework enhances the likelihood of successfully implementing a PBRN's mission.
