Area of Science:

  • Otolaryngology
  • Laryngology
  • Surgical Innovation

Background:

  • Bilateral vocal cord palsy causes significant respiratory distress.
  • Various surgical interventions exist, but outcomes vary.
  • Laser CO(2) posterior cordectomy is a proposed treatment option.

Purpose of the Study:

  • To evaluate the efficacy of laser CO(2) posterior cordectomy.
  • To assess improvements in dyspnea and voice quality.
  • To analyze the safety and effectiveness of the procedure.

Main Methods:

  • Retrospective study of 13 patients (9 female, 4 male) aged 25-79.
  • Most common etiology: iatrogenic post-thyroidectomy palsy.
  • Voice Handicap Index (VHI) used to assess subjective outcomes.

Main Results:

  • All patients experienced improved dyspnea post-surgery.
  • Two cases had immediate postoperative dyspnea worsening, one resolved with re-intervention.
  • Most patients reported mild to moderate dysphonia after the procedure.

Conclusions:

  • Posterior cordectomy is a safe and effective treatment for dyspnea in bilateral laryngeal palsy.
  • The procedure offers significant relief from respiratory distress.
  • Acceptable voice quality is maintained post-surgery.
