Background:

  • Multifocal choroidopathy syndromes are rare disorders affecting the retinal pigment epithelium and choriocapillaris.

Observation:

  • A 54-year-old woman presented with blurred vision, ocular pain, and visual disturbances.
  • Anterior segment examination revealed keratic precipitates and anterior chamber inflammation.
  • Fundus examination showed characteristic yellow-white spots at the retinal pigment epithelium and choriocapillaris level.

Findings:

  • Fluorescein angiography demonstrated active lesions with blocked early choroidal fluorescence and late staining, and inactive lesions with RPE window defects.
  • Laboratory tests indicated elevated blood glucose, C-reactive protein, and lipids.
  • Immunological examination revealed low C3 and C4 complement components and elevated immune complexes.
  • Serological tests were positive for Cytomegalovirus, Herpes Simplex Virus, and Varicella Zoster Virus IgG.

Implications:

  • The presence of multiple viral infections and immunological abnormalities suggests an autoimmune mechanism potentially triggered by viral agents.
  • This case highlights the importance of comprehensive laboratory and immunological workup in diagnosing multifocal choroidopathy syndrome.
