Summary

This study compared breast reconstruction methods after mastectomy, finding that free perforator flaps, particularly the Deep Inferior Epigastric Perforator (DIEP) flap, offer promising aesthetic results with manageable complications.

Area of Science:

  • Plastic Surgery
  • Oncologic Reconstruction
  • Aesthetic Surgery

Background:

  • Breast reconstruction (BR) aims to restore symmetry and quality of life after mastectomy.
  • Evaluating various BR techniques is crucial for optimizing patient outcomes.

Purpose of the Study:

  • To assess the aesthetic outcomes of different breast reconstruction methods.
  • To document and analyze complications associated with various BR procedures.

Main Methods:

  • Retrospective review of 36 women undergoing BR post-mastectomy (August 2006-March 2010).
  • Immediate BR utilized for Stage I-IIa breast carcinoma; delayed BR for Stage IIb-III.
  • Surgical techniques included Transverse Rectus Abdominis Muscle flap, Latissimus Dorsi flap with implant, and Deep Inferior Epigastric Perforator (DIEP) flap.

Endoscopic Bilateral Nipple-sparing Mastectomy via a Single Axillary Incision with Immediate Pre-pectoral Implant-based Breast Reconstruction

Published on: May 17, 2024

Main Results:

  • Commonly performed procedures: Transverse Rectus Abdominis Muscle flap (13), Latissimus Dorsi with implant (10), DIEP flap (9).
  • Complications observed: flap failure (1), partial flap necrosis (3), wound dehiscence (1), seroma (1), local infection (1).

Conclusions:

  • Free perforator flaps, especially DIEP, are increasingly favored for breast reconstruction.
  • Immediate BR shows superior aesthetic results when indicated.
  • Autologous procedures are a viable option, particularly where implants are not covered by insurance.
