UK urological trainees show a decline in performing transurethral resection of the prostate (TURP), radical nephrectomy (RN), and radical cystectomy (RC). Significant variability in operative experience persists among trainees.
Area of Science:
- Urology
- Surgical Education
- Operative Experience
Background:
- Surgical training emphasizes developing operative skills through practical experience.
- The operative experience of urological trainees in the UK has not been previously documented.
- Understanding current trends in surgical experience is crucial for effective training.
Purpose of the Study:
- To examine the operative experience of UK urological trainees over a six-year period (2004-2009).
- To analyze changes in the volume of specific procedures performed by trainees.
- To identify potential reasons for observed changes and discuss implications for future urological training.
Main Methods:
- Analysis of operative logbooks submitted by urological trainees for Certificate of Completion of Training.
- Inclusion of 251 logbooks from 2004 to 2009.
- Data collection on trainee experience in eight key procedures: TURP, TURBT, RN (open and laparoscopic), RC, RP, PCNL, and ureteroscopy.
Main Results:
- Significant reduction in the number of transurethral resection of the prostate (TURP), open radical nephrectomy (RN), and radical cystectomy (RC) performed by trainees.
- Increased use of trainees as assistants for radical cystectomy (RC), radical prostatectomy (RP), and open radical nephrectomy (RN).
- Substantial variation in the number of procedures performed across individual trainees for all analyzed operations.
Conclusions:
- A recent decline in the performance of TURP, open RN, and RC procedures by urological trainees has been observed.
- Considerable variability in operative case numbers among trainees highlights potential disparities in training.
- The findings necessitate a review of training strategies to ensure adequate surgical skill development for future urologists.
