Summary

Acromial fractures after reverse shoulder arthroplasty (RSA) often do not unite but typically do not cause persistent pain. Nonoperative management is a reasonable option for this complication, despite reduced shoulder function.

  • Eight patients (4.9%) experienced acromial fractures post-RSA.
  • Six of eight fractures did not unite; functional outcomes included mean active forward elevation of 71° and mean ASES score of 70.
  • Most patients reported no or mild pain, despite nonunion; 74% of surveyed surgeons favored nonoperative management.
  • Conclusions:

    • Acromial fractures after RSA have a high rate of nonunion and may lead to reduced shoulder function.
    • Despite nonunion, most patients do not experience chronic pain, supporting conservative management as a viable option.
    • Surgeon opinion aligns with conservative management, emphasizing functional decline over persistent pain for this complication.
