Summary

Transcranial sonography (TCS) detects basal ganglia abnormalities in Wilson's disease (WD). This study found increased substantia nigra (SN) and lentiform nucleus (LN) hyperechogenicity in WD patients, correlating with disease severity.

Background:

  • Transcranial sonography (TCS) is a sensitive tool for detecting basal ganglia (BG) abnormalities in movement disorders.
  • Previous research indicates patterned hyperechogenic lesions in BG in various conditions.

Purpose of the Study:

  • To investigate TCS changes in a larger cohort of clinically stable Wilson's disease (WD) patients.
  • To correlate TCS findings with demographic and clinical data in WD.

Main Methods:

  • TCS was performed on 54 consecutive, clinically stable WD patients.
  • Patients were categorized into neurologic or hepatic forms.
  • TCS findings were compared to healthy controls.

Main Results:

  • WD patients showed a significantly higher prevalence of substantia nigra (SN) and lentiform nucleus (LN) hyperechogenicity compared to controls.
  • Moderate to marked SN hyperechogenicity was observed in 31.5% of WD patients.
  • Disease severity correlated with SN hyperechogenicity and third ventricle width.

Conclusions:

  • TCS is effective in detecting BG abnormalities in WD patients.
  • Findings suggest TCS can identify copper and other trace metal accumulation in WD.
  • TCS findings correlate with disease severity and specific clinical forms of WD.
