A rare complication during percutaneous mitral valvuloplasty, balloon rupture, led to air embolism in the coronary artery. Prompt intervention successfully cleared the air, restoring blood flow and stabilizing the patient.

- Restoration of TIMI grade 3 flow and myocardial blush was confirmed.
