Mucosal intussusception effectively prevents ascending cholangitis after Kasai

Area of Science:

  • Gastroenterology
  • Pediatric Surgery
  • Surgical Innovation

Background:

  • Ascending cholangitis is a complication of Kasai's hepatic portoenterostomy for biliary atresia.
  • Preventing bacterial reflux into the biliary system is crucial after this procedure.

Purpose of the Study:

  • To evaluate the efficacy of mucosal intussusception in preventing ascending cholangitis.
  • To assess the impact of mucosal intussusception on bacterial reflux in a canine model.

Main Methods:

  • Twenty mongrel dogs underwent Roux-en-Y cholecystjejunostomy and common bile duct ligation.
  • Dogs were randomized to receive either mucosal intussusception or no intussusception.
  • Aeromonas hydrophila was instilled into the jejunum; bacterial cultures and barium studies were performed.

Related Experiment Videos

Main Results:

  • Mucosal intussusception prevented reflux in all treated dogs.
  • Significantly fewer gallbladder cultures were positive for Aeromonas in the intussusception group (P < 0.05).
  • Control dogs showed a high rate of positive gallbladder cultures.

Conclusions:

  • Mucosal intussusception is a promising technique for preventing ascending cholangitis.
  • This method warrants clinical trials for preventing or treating post-Kasai cholangitis.
