Background:

  • Anti-p200 pemphigoid is a rare autoimmune subepidermal blistering disorder.
  • It can clinically mimic other blistering conditions like bullous pemphigoid.
  • Characterized by IgG antibodies against a 200 kDa basement membrane zone protein.

Observation:

  • Presents the first known case of anti-p200 pemphigoid associated with IgA antibodies.
  • Clinical presentation resembled pemphigus herpetiformis or dermatitis herpetiformis on traumatized skin.
  • Histopathology revealed dermal-epidermal separation and dermal papillary microabscesses.

Findings:

  • Direct immunofluorescence showed linear in vivo-bound IgA at the basement membrane zone.
  • IgA antibodies were localized above collagen type IV and colocalized with laminin 332.
