Summary

Physicians unfamiliar with chilblains (perniosis) may lead to unnecessary hospitalizations and treatments. Early recognition of this cold-induced skin condition can prevent costly and potentially harmful interventions.

Background:

  • Chilblains (perniosis) are often misdiagnosed, leading to inappropriate medical interventions.
  • Physician unfamiliarity contributes to delayed diagnosis and management of this condition.

Observation:

  • Seven cases of chilblains were observed in San Francisco between November 1986 and January 1987.
  • Patients presented with itchy, painful, purple lesions on extremities and nose following cold, damp exposure.

Findings:

  • Histopathology revealed perivascular lymphocytic infiltrate and endothelial swelling in subcutaneous fat and dermal plexus.
  • Clinical presentation includes pruritic, painful, purple acral patches after cold exposure.

Implications:

  • Improved physician awareness of chilblains can reduce unnecessary hospital admissions and evaluations.
  • Timely diagnosis and appropriate management can prevent hazardous therapies and healthcare costs.
