Background:

Neurothekeoma is a recently identified benign cutaneous tumor originating from the nerve sheath.
It is histologically distinct from other nerve sheath tumors like neurofibroma and neurilemmoma.
Dermal nodules require accurate histopathological diagnosis for appropriate management.

Observation:

A 62-year-old male presented with a 6-month history of a slowly growing, asymptomatic 10-mm dermal nodule on his ankle.
Initial histological examination revealed extensive dystrophic calcification within the tumor lobules.
The calcification pattern mimicked a foreign body granuloma, causing diagnostic uncertainty.

Findings:

Histological analysis confirmed the nodule as a neurothekeoma.
