Late microsurgical flap failure can occur due to rare proximal vessel occlusion. This case highlights the importance of considering external iliac artery occlusion in patients with peripheral vascular disease risk factors.
Area of Science:
- Vascular Surgery
- Microsurgery
- Reconstructive Surgery
Background:
- Late microsurgical flap failure is uncommon, typically linked to vascular pedicle compression or infection.
- Peripheral vascular disease poses risks for reconstructive surgery outcomes.
Observation:
- A case of partial microsurgical flap failure occurred 3 weeks post-operatively.
- Vascular imaging revealed a previously unknown complete occlusion of the right external iliac artery.
Findings:
- Successful vascular bypass surgery restored blood flow to the compromised flap.
- The flap subsequently developed granulation tissue and was managed with a skin graft.
Implications:
- In patients with multiple peripheral vascular disease risk factors, proximal vessel occlusion must be considered as a cause of late flap failure.
- This case underscores the need for thorough pre-operative assessment and vigilance for uncommon vascular complications in reconstructive surgery.

Flail Chest-I
Flail chest is a severe and potentially life-threatening condition characterized by the fracture of three or more adjacent ribs in multiple places. It is most commonly caused by direct impacts and trauma, such as motor vehicle accidents or injuries from a steering wheel impact. It can also occur due to falls in elderly individuals with osteoporosis, or assaults involving sharp objects.
Pathophysiology
The pathophysiology of flail chest is complex, involving fractures of...
Acute Respiratory Failure-I
Definition: It is defined by specific criteria based on blood gas measurements. Hypoxemia happens when the partial pressure of oxygen (PaO2) falls below 60 mmHg. At the same time,...
