Background:

Alloplastic malar augmentation is a common procedure for midface enhancement.
It is also used to improve outcomes of other facial rejuvenation procedures.

Purpose of the Study:

To present a novel intranasal surgical approach for malar implant placement.
To introduce a new classification system for midface regions to aid augmentation planning.

Main Methods:

A cohort of 20 patients underwent alloplastic malar augmentation using an intranasal approach between 1990 and 2010.
Patients were categorized into three types based on nasal anatomy and need for alar correction.
Implants were selected and placed according to the proposed classification and surgical technique.

A Standardized Surgical Technique for Tessier Medial Transnasal Canthopexy

The study included 18 females and 2 males, aged 15-65 years, with an average follow-up of 10 years.
All patients achieved favorable results with no major complications, nerve/vascular compromise, or implant malposition.
Patient satisfaction was high at 95%, with only one patient requesting implant removal despite a good outcome.

Conclusions:

The intranasal approach for alloplastic malar augmentation is effective for midface enhancement.
This technique demonstrated excellent patient satisfaction and a very low complication rate in this series.
