Summary

Salvage chemotherapy with pemetrexed in non-small cell lung carcinoma (NSCLC) showed activity in real-world practice. However, significant toxicity, including neutropenia, was observed in heavily pre-treated patients.

Area of Science:

  • Oncology
  • Clinical Pharmacology

Background:

  • Pemetrexed demonstrated efficacy in non-small cell lung carcinoma (NSCLC) in clinical trials.
  • Its use in salvage therapy for NSCLC is established.
  • Real-world validation is crucial for unselected patient populations.

Purpose of the Study:

  • To evaluate the effectiveness and safety of pemetrexed salvage chemotherapy in an unselected NSCLC patient population in clinical practice.
  • To compare real-world outcomes with those from registration trials.

Main Methods:

  • Retrospective chart review of 84 NSCLC patients receiving pemetrexed after prior chemotherapy.
  • Assessment of response using WHO criteria and toxicity via CTCAE and WHO criteria.
  • Analysis of second, third, and later-line treatment outcomes.

Main Results:

  • An intent-to-treat response rate of 11.9% was observed.
  • Median progression-free survival was 2.2 months; median survival time was 6.4 months.
  • Neutropenia was the most frequent grade 3-5 toxicity, occurring in 23.9% of patients.

Conclusions:

  • Pemetrexed salvage chemotherapy confirms activity in extensively pre-treated NSCLC patients in a real-world setting.
  • The observed activity aligns with registration trial findings.
  • Significant toxicity, particularly neutropenia, necessitates careful monitoring.
