Area of Science:

  • Endocrine Surgery
  • Surgical Innovation
  • Thyroid Disease Management

Background:

  • Conventional ligation for thyroidectomy carries risks of post-operative complications.
  • Limited evidence exists on the comparative safety of advanced vessel sealing devices in thyroidectomy.
  • This study addresses the need for adequately powered research on sutureless techniques.

Purpose of the Study:

  • To compare sutureless thyroidectomy with conventional methods.
  • To evaluate outcomes including recurrent laryngeal nerve (RLN) injury, hypoparathyroidism, and hematoma.
  • To assess operative time, vocal cord dysfunction, and cost-effectiveness.

Main Methods:

  • A cohort study compared 1163 sutureless thyroidectomies with 772 conventional thyroidectomies.

  • Sutureless procedures utilized Ligasure Precise or Harmonic Focus devices.
  • Primary outcomes included hemorrhage, permanent RLN injury, and permanent hypoparathyroidism.
  • Main Results:

    • Mean operative time was significantly lower in the sutureless group (71 vs. 86 min, P=0.02).
    • No significant differences were observed in hematoma, permanent hypoparathyroidism, or permanent RLN injury rates.
    • Sutureless thyroidectomy demonstrated an overall cost saving of AUD$14,300 per 100 cases.

    Conclusions:

    • Sutureless thyroidectomy is a safe and effective alternative to conventional techniques.
    • The method offers potential for reduced operating room costs.
    • This approach is suitable for total thyroidectomy procedures.
