Background:

  • Individualizing lung cancer treatment requires understanding long-term survival predictors.
  • Diffusing capacity (DLCO) is known to predict complications after lung cancer resection.

Purpose of the Study:

  • To determine if diffusing capacity (DLCO) is also prognostic for long-term survival in patients undergoing lung resection for cancer.

Main Methods:

  • Survival analysis of 854 patients who underwent lung resection for cancer (1980-2006).
  • Evaluation of prognostic factors for all-cause mortality using hazard rates and multivariable Cox modeling.
  • Multiple imputation was used to handle missing data.

Main Results:

  • DLCO was identified as an independent predictor of long-term survival.
  • On multivariable analysis, a 10-point decrement in DLCO was associated with a 4% increased hazard of mortality (HR 1.04; P = 0.05).
  • Prognostic ability of DLCO extended beyond its impact on operative mortality.

Conclusions:

  • Diffusing capacity (DLCO) is a clinically significant, independent determinant of long-term survival post-lung resection for cancer.
  • Findings can inform patient selection for surgery and help tailor resection extent to balance operative risk and long-term outcomes.
