Summary

Adding horizontal grooves to the internal surface of loose metal crowns significantly increases their retention. This simple modification enhances crown stability without needing a new crown or extensive tooth preparation.

  • To evaluate methods for increasing the retention of loose cast metal crowns.
  • To determine if retention can be improved without remaking the crown or altering the tooth preparation.

Main Methods:

  • Ninety cast metal crowns were fabricated with slight internal looseness.
  • Horizontal grooves were introduced on internal crown surfaces and/or external die surfaces.
  • Crowns were cemented and tested for dislodgement using tensile force.
  • Statistical analysis (ANOVA, t tests) was performed to compare retention.

Main Results:

  • A single horizontal groove on the internal crown surface significantly increased retention (P<.001).
  • Two internal grooves further enhanced retention significantly (P<.001).
  • Grooves on the die or both crown and die offered no significant additional benefit over internal crown grooves.

Conclusions:

  • Incorporating 1-2 horizontal circumferential grooves internally enhances cast metal crown retention.
  • Internal crown grooving is an effective method to improve retention, comparable or superior to die grooving.
